Non refundable

Refunds are generally not provided. However, in exceptional cases, you may request a refund, which will be considered on a case-by-case basis. Approval of such requests is at the sole discretion of EVOLVE HC. Requests must be made in writing and submitted to EVOLVE HC for review.

Full liability waiver

You agree to release EVOLVE HC from any liability for any loss, damage, or injury that may occur while receiving the services. You understand that participating in any activity or service provided by EVOLVE HC involves certain risks and accept full responsibility for your safety and well-being.

Information sharing restricted

You agree not to share, distribute, or disclose any confidential information obtained through interactions with EVOLVE HC, except as mutually agreed in writing. Unauthorized sharing of such information will be directly addressed by EVOLVE HC

Once purchased, Emma will send a new client assessment via email & please allow up to 5 business days for your plan to be sent to you after the assessment is complete.
